The NHS Health Check is your chance to get a free midlife MOT.
Appointments are now bookable online (registration required - see Patient Access) but please check you meet the eligibility criteria first (appointment bookings will be checked and appointments cancelled where patients are ineligible):
*Patients known to have cardiovascular disease are reviewed separately and are not eligible for this check. This includes patients diagnosed with hypertension (high blood pressure) and patients taking statins.
The NHS Health Check takes 30 minutes and is designed to check your circulatory and vascular health to assess your risk of developing heart disease, stroke, kidney disease or diabetes. There is good evidence that by taking early action on any warning signs noted at the Health Check, you can improve your health and prevent the onset of these conditions.

The check involves straightforward questions and measurements such as age, sex, family history, height, weight and blood pressure. There will also be a simple blood test to measure your cholesterol level. Following the check, you will receive free personalised advice about what you can do to stay healthy.
It will save time in your appointment if you could download the below questionnaires to complete in advance and bring with you:

We send out postal, email and/or text message invitations but it will necessarily take
a while to invite all our eligible patients.
If you are in the target age range but have not received an invitation
letter please telephone and a Receptionist will check if you are eligible
before booking an appointment if appropriate.
If you do not want a health check, we would be grateful if you could let us know to save us contacting you. Please note that once you have had a Health Check you will not be eligible to receive a Health Check again for five years.
