Cancelling your Appointment
If you are unable to attend an appointment with one of the doctors or nurses, please telephone or use the link at the bottom of this page to cancel your appointment.
By giving us as much notice as you can you are helping us to make sure that someone else is given your slot.
Appointments may be booked either in person, by telephone or on-line.
There is a huge demand for GP appointments. Before booking a routine GP appointment, please read the guidelines below to help you judge what appointment, if any, is required:
Appointments are available at either of our sites for all patients. We also work together with other local practices to offer more appointments in the early mornings, evenings and at weekends.
Routine GP appointments can be pre-booked up to 2 weeks in advance. We encourage you to see the same doctor for on-going problems.

Nurse appointments can be booked up to 6 weeks in advance.
All patients who wish to be seen urgently will usually be offered a telephone triage call from the Urgent Care Team. By using a triage service we are able to ensure that clinically urgent problems are prioritised - urgent problems will be seen within 24 hours. Urgent care appointments are provided at both sites however depending on the level of demand at each site, patients may need to be flexible and attend either surgery.
To help the triage Doctor prioritise calls, please provide a brief reason for needing to see/speak to a Doctor. If the Doctor assesses the problem as non-urgent, you may be offered telephone advice, medication or advised to book a routine appointment to assess the problem.
We are very aware that our patients lead busy lives. We now offer booked telephone appointments with a doctor. Many problems that don't require physical examination can be successfully dealt with over the telephone.
We encourage patients to attend the surgery where possible however home visits are available if you are housebound or too ill to come to the surgery - please telephone before 10am. Reception will ask for details to allow the doctors to assess the urgency. A doctor may telephone to obtain more information and establish whether a home visit is required.
Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health Centre.
If you cannot attend an appointment for any reason please inform us as soon as possible, this reduces the wait for appointments for everyone. All appointments can be cancelled online (if registered), whether booked online or not.
We send SMS (text message) and email appointment reminders where possible - please ensure we have your correct mobile number and/or email details. If you would like to opt out of text message reminders, please contact us.
You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.
If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay).
It is up to your employer to decide whether you are incapable of work. A medical certificate, now called a 'Statement of Fitness for Work’ (see below) from your doctor is strong evidence that you are sick and would normally be accepted, unless there is evidence to prove otherwise.
You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist. Your employer will decide whether or not this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP.
The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury.

Please contact us in the month before your birthday to book all Long Term Conditions review appointments during the month of your birthday. You may need to see more than one clinician (GP and/or Nurses) however we will combine appointments where we can. We hope this will make it easy for you to remember and to get your health fully reviewed in a timely fashion.

An annual review is also advisable if you have Peripheral Arterial Disease (PAD) or Chronic Kidney Disease (CKD) however these diseases are often secondary to other long term conditions such as Diabetes so you may not be aware that you have been diagnosed with these conditions or that we have been monitoring them.
You will be advised at your annual review to book additional appointments if your condition needs further management. We no longer advise you to routinely book interim checks, such as mid-year checks of HbA1c for diabetics or mid-year blood pressure checks.
Our receptionists will be able to advise you what Long Term Conditions you have been diagnosed with and what appointments you will require. Many conditions need annual blood tests – these should be combined into one 10 minute appointment with a Health Care Assistant (HCA) approximately two weeks before your review appointment.
If you are transitioning to this system from a non-birth month recall date, there may be a shorter or longer than usual gap from your last review to your birthday month. Our receptionists will be able to advise you of the best date to book your next review, dependent on your birth month and last review date, or see our guidance for 2018/19 (1 page pdf)
If you have an administrative request, eg you need a GP letter, a holiday cancellation form completed by a GP, information for Blue Badge or Army applications, please download and complete the below form and return to us together with any original paperwork relating to your request.
