All patients, including children, are allocated a named, accountable GP, referred to as your 'Usual GP' - you should be advised who this is when you register. We will make reasonable efforts to accommodate requests to change Usual GP.
We require BOTH a Registration form AND a Medical Questionnaire to be completed in order to register you.

On receipt the Admin team will process your registration - please allow two working days for this, then you will be able to make an appointment with us if needed.

Organ & Blood Donation
Although the back of the GMS1 form includes options to register to be an organ and/or blood donor, this can no longer be acted on locally.

If you are ill while away from home or are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days.
After 14 days you will need to register as a temporary or permanent patient. Temporary registration lasts up to three months and allows you to be treated here and still remain a patient of your permanent GP practice.
We only register patients temporarily if absolutely necessary - many issues will be able to be dealt with over the phone with your permanent GP.
Once You Are Registered With the Practice
This practice is signed up to the Electronic Prescription Service (EPS) - if you have previously nominated a pharmacy which is no longer convenient for you to attend, please ensure you update your nomination by telling us or your new pharmacy.
Please ask at Reception if you would like to register for Online Services which will enable you to book appointments and request repeat medication online.
If you were set up for online access with your previous GP practice this will not automatically transfer. Once you request new registration information from us you will however be able to use this to add our practice to your existing account, so you can use the same log-in details.
Your details will be added to our computer system and your patient record will be kept electronically on our practice systems. We advise you read our information about Confidentiality and your Patient Record - this includes details about how to opt out of sharing your data if you have concerns on this matter.
We encourage you to see your Usual GP where possible, particularly for on-going problems, so we can provide you with better continuity of care. If your Usual GP is fully booked, you can ask for an appointment with another Doctor or a Minor Illness Practitioner.
We do not ask new patients to make an initial appointment for a general health check.
